The undersigned Applicant(s) has applied for Net (21) Terms at National Paper and Sanitary Supply. You are hereby authorized to release any information to National Paper and Sanitary Supply, which is required to complete the processing of this application request.

Necessary information may include, but is not limited to: (1) checking account verification, (2) loan status and payment history verification, (3) if Applicant has been denied credit within prior 12 months; and (4) any other financial verification.

In addition, a credit report may be obtained to facilitate the processing of the application.
